You can send a written request to the Fremont Police Department, along with payment, mailing address, stamped envelope, and a copy of your government-issued photo ID.

Fremont accident reports are $16 per copy.

• The names and contact information for all parties involved in the motor vehicle accident

• The reporting law enforcement officer’s opinion concerning who they believe was at fault for the crash and how and why the accident occurred

• Name of responding law enforcement agency
