To request paperwork, you must include a self-addressed envelope, the completed agency form, a copy of your current government-issued photo ID, and the appropriate fee for mail delivery.

Fort Worth accident reports will cost $10 for the first ten pages and $0.25 for each additional page. Accepted forms of payment include business checks, money orders, personal checks, credit cards, or debit cards. 

A crash report will contain the following information:

• The names and contact information for all parties involved
• A description of the accident scene

An accident report is often filed by the responding police officer within seventy-two hours following an accident. However, it can take up to ten business days before the report is available for pick up or is mailed, if the patrol officer is still investigating the accident.
