In Person

You can visit the California Highway Patrol Station in Baldwin Park and fill out a CHP 190 Collision Request Form. You must show your valid ID and pay the accident report fee at the time of pick up. The station is located at:14039 Francisquito AvenueBaldwin Park, CA91706

By Mail

You must visit the CHP website and print off the CHP 190 Collision Request Form and follow the form’s instructions. You must provide the date and location of the accident, the name of the driver or owner of the vehicle involved, include all applicable fees and a self-addressed, stamped envelope, and a copy of your valid photo ID.

CHP accident reports can cost $10.00 to $40.00 for reports up to 100 pages.
