The Peoria Police Department offers two ways to get your accident report.

In Person

The Peoria Police Department Records Section is responsible for maintaining and distributing accident reports produced by police officers at the Peoria Police Department. You can print off the Records Request Form found on the department’s website and bring it to: Peoria Police Department Records Unit8351 W. By Mail

The other option is to fill out the Records Request Form and mail it, along with a fee, to:8351 W. Cinnabar AvePeoria, AZ 85345Payment can be made by money order or check, made payable to the City of Peoria. The police department does not send documents via email or fax.

At the scene of the accident, the responding police officer will fill out a Peoria accident report form that includes important information regarding the accident, such as a description of the damage done to all vehicles involved, the names of both drivers, the time, date, and location of the accident, and the officer’s opinion regarding who they believe is at fault for the accident. Once you’re finished speaking with the police officer, you’ll receive an information exchange sheet, which includes a report number. You’ll use the report number the police officer gave you to order a copy of your report from the Peoria Police Department.

Peoria or Tempe police reports will cost $8.00 for the first ten pages, with $0.15 for each additional page.

Reports that involve domestic violence, theft, or assault may not be immediately available if there is an active investigation.
