To receive your report by mail, you must send a complete Crash Report Request Form and include a money order or certified check made out to the Alabama Department of Public Safety. Mail your request to:Alabama Law Enforcement AgencyCrash ReportsP.O. A crash report will contain information including:

• The date, time, and location of the accident
• Contact details for all parties involved
• Description of personal injuries and property damage
• The crash narrative, which includes the opinion of the police officer concerning who they believe is the at-fault driver.
• Auto accident type (scooter accident, rollover, head-on collision, motorcycle accident, semi-truck accident, etc.)
• Any citations issued or arrests made (DUI, DWI, speeding, reckless driving, failure to follow traffic laws, etc.)
• The of the responding law enforcement agency and the reporting police officer’s name and badge number
• Driver’s license numbers
• VINs
• Insurance policy information
• License plate numbers
• A diagram of the crash site
• Vehicle registration information

You must allow a minimum of seventy-two hours for processing time before the report is completed by the responding law enforcement officer. Reports are usually filed within this time unless the accident involved life-threatening injuries, a fatality, or a crime. If there is an open investigation, the report may be unavailable for several weeks or months. It may take up to ten business days from the date the accident report was filed before you receive your report in the mail.

If you have the accident report reference or ID number available, be sure to include that on the request form. You will also need to include the following information:
