The Illinois State Traffic Report SR 1050 was designed by IDOT, also referred to as the Illinois Department of Transportation, and documents a law enforcement agency member’s investigation of a motor vehicle collision. This form contains crash data, detailing the accounts of the auto accident, automobile accident type, and other relevant motor vehicle crash information.

You can use the information center available at AccidentReports.com or you can obtain a copy of your traffic accident report from the Illinois State Police Department.The Illinois State Police Department offers three ways to obtain your crash report.

Accident Report Requests By Email

You can visit the Illinois State Police website and fill out an online Records Request Form. You must also pay a fee at this time, depending on the length of the report. You will receive your report once it’s ready. Unfortunately, due to the high volume of requests, it can take several business days or weeks before you receive your copy.

In-Person

Request Traffic Crash Reports By Mail

When you request an Illinois traffic accident report you must provide the following information:

• Vehicle registration information
• Date, time, location of the accident
• Full name of one of the drivers involved
• The collision report number (if available)
• County in which the accident occurred
• Description of any bodily injury
• Description of property damage
• Reporting law enforcement agency

All of this information will make it easier for the Records Unit staff to locate your motorist report.

• The time, date, and location of the accident
• Insurance policy information
• The names and contact details of all parties involved
• VIN, make, model, of each vehicle
• Vehicle registration information
• License plate numbers for each vehicle involved
• Anything that may have contributed to the crash (debris on the road, poor road conditions, traffic congestion, inclement weather, etc.)
• Accident type (motorcycle accident, pedestrian accident, semi-truck accident, head-on collision, etc,)
• Cause of accident (speeding, drunk driving, failure to yield, etc.)
• A description of personal property damage
• A description of any personal injuries sustained in the crash
• The name of the reporting law enforcement agency and police officer’s name and badge number
• A diagram of the accident scene
• Police officer’s opinion regarding who was liable for the accident and why the accident occurred
• Any citations issued or arrests made ( speeding, DWI, DUI, etc.)
• Witness statements and contact information

In most cases, it will take up to ten business days for the State Police Department to process your record request. It may take longer if the traffic crash involved bodily injury, fatalities, significant property damage, a crime took place, or there is an open investigation.

An Illinois police report is $5.00. For Email requests, there is an additional $1.00 processing charge. For mail or in-person requests, payments must be made by money order or check.
